HISTORY ON REQUISITIONS

Including patient history that matches the requested exam is important when completing a
requisition. If the history doesn’t match the exam being ordered or if the history is incomplete,
bookings may be delayed until we can connect with you to confirm details.
Please include the relevant history related to the exam for a speedier booking process. If multiple
exams are requested, please include history for all exams. Per CPSS regulations, we also require
the patient’s provincial health care number be included on the requisition to book the exam.

REQUESTS FOR REPEAT PROCEDURES

For repeat procedures, we kindly ask that you update the requisition with the most current and
accurate information, including the correct dates. Please avoid reusing a previous requisition, as it
may be outdated and no longer valid.

STAY CURRENT WITH REQUISITIONS

We regularly update our requisitions to improve clarity and support more efficient patient care.
You can download the updated requisitions for integration into your EMR system. If you require
printed copies, please contact us. Visit physicians.radiology.ca for the most current versions.

35 LANGUAGES AVAILABLE ON PATIENT WEBSITE

Our patient website, radiology.ca, is now available in 35 different languages. Patients can select
their preferred language to access exam prep instructions, explanation of services, descriptions of
exams, directions for our locations, as well as health and wellness articles. Patients can access this
feature from the top left-hand corner of our homepage at radiology.ca.
