IMPORTANCE OF HISTORY ON REQUISITIONS
Including patient history that matches the requested exam is important when completing a requisition. If the history doesn’t match the exam being ordered or if the history is incomplete, bookings may be delayed until we can connect with you to confirm details. Typically, we will fax the requisition back to you for modifications. Please include the relevant history related to the exam for a speedier booking process. If multiple exams are requested, please include history for all exams.
We also require the patient’s provincial health care number be included on the requisition to book the exam.
LAB ASPIRATION REQUISITIONS REQUIRED
When referring a patient for an exam with an aspiration, if you require the aspiration to be sent to a lab, we will require not only the exam requisition but also a lab requisition for the aspiration. Without the lab requisition, we are unable to send the aspiration to the lab.
SECOND PAGE OF REQUISITIONS
When printing a requisition for a patient at your clinic, when possible, please print both the first page and second page. The second page includes important information for the patient to know prior to their exam. You can find our requisitions and prep instructions at physicians.radiology.ca.

TRANSLATORS FOR PATIENTS
Patients receiving pain management treatments who need language interpretation or prefer to communicate in a language other than English are encouraged to bring a family member or friend to their appointment to assist with translation. This ensures that patients fully understand the procedure and necessary instructions. If the patient is unable to comprehend the instructions, the appointment may need to be rescheduled. As Mayfair does not provide language interpreters, it is essential for patients to make these arrangements in advance.
MULTIPLE LANGUAGES AVAILABLE ON PATIENT WEBSITE
Our patient website, radiology.ca, is available in 15 different languages for your patients. Patients can select their preferred language to access information such as prep instructions, explanation of services, descriptions of exams, directions for our locations, as well as health and wellness articles. Patients can access this feature from our homepage at radiology.ca.
